Similar to Mario Carreon , just secondhand anecdotal experience from my doctor friend. He happened to be studying the disease of the episode and guessed it correctly before the show's characters did.

A lot of the time frame is rushed. Same question applies to forensics ppl & CSI. There was one news feature on a doctor who diagnosed a patient based on a House ep.
